Description (Service) COVID19 Supporting over phone, in person as needed.

Provide eligibility assessment, coordination of care, placement services and system navigation for those in need of community-based health services. Assessment for in-home services and placement also provided, in-home services include nursing, nutrition services, occupational therapy, physiotherapy, social work, speech-language pathology, personal support and homemaking services. Placement services can include placements into Long-Term Care Homes for both long-term or short-term and adult day programs, rehabilitation and complex continuing care beds. Access to assisted living and supportive housing

Inquiries/Support: Mon-Sun 8 am-8 pm (toll free and TDD numbers)

Application Self referral for most services. Referral through school for in-school services
